WOUND MANAGEMENT PROGRAM

Program Philosophy

Wound Care Services and Treatments

CareResource Hawaii's wound management program program provides therapies, services and care for clients who require treatment and resolution of wounds, demonstrating timely and complex healing processes. Those individuals evaluated for admission to this program may be experiencing any of the following:

  • Wounds associated with vascular impairment
  • Pressure wounds
  • Traumatic wounds
  • Post-surgical wounds
  • Burns
  • Chemical wounds
  • Radiation wounds
  • Diabetic ulcers
  • Wound infections, including pseudomonas, MRSA, and VRE
  • Urinary / fecal diversions
  • Venous stasis ulcers

A Staff of Specialists

In collaboration with the client's physician and drawing upon a broad spectrum of medical and therapeutic specialties, we use a goal-directed approach to help clients realize their potential for independence in daily living. Each client's needs are continually monitored to ensure that the most appropriate level of care is provided. Our staff includes a diversified team of the following:

  • Certified Enterostomal Therapist
  • Clinical Nurse Specialist / Coordinator
  • Registered Nurses
  • Physical Therapist
  • Occupational Therapist 
  • Speech / Language Pathologist
  • Registered Dietitian Consultant

Family Support - a Crucial Component

Family support and education is provided at every opportunity. We recognize that a patient's "family" includes all individuals who, by birth or choice, make up the patient's support network. Our role is to involve the family by giving advice and encouraging open discussions about:

  • Current health status
  • Treatment options available and potential side effects
  • Components of self-care and home environment
  • Lifestyle adaptations that may be necessary
  • Discharge planning and preparation
