REHABILITATION PROGRAM

Program Philosophy

When illness or injuries require rehabilitative and restorative therapies concurrent with or subsequent to other medical care, home care can provide the solution to complete the continuum of care. Home care transitions the client to an increasingly independent lifestyle by providing appropriate care, skills training, and adaptive equipment in the home environment.

Under the direction of the client's physician, the experienced and dedicated rehabilitation staff of CareResource Hawaii provides a full spectrum of services to ensure the delivery of client-specific, high quality care through our transdiciplinary case management approach. Our therapists have access to the latest technology and equipment. We are accredited by the Joint Commission on the Accreditation of Healthcare Organizations.

Possible Diagnostic Categories

Numerous conditions can be treated through rehabilitative and restorative therapies in the home. Diagnoses can include:

• Congenital or acquired neurological disorders such as cardiovascular accident, spinal cord injury, head trauma, neuropthies and spina bifida
• Cardiac disease
• Cerebral vascular accident
• Respiratory disease
• Orthopedic conditions such as fractures, joint replacement and amputation
• Musculoskeletal and connective tissue disorders such as amytrophic lateral sclerosis, multiple sclerosis, and polyarthritis
• Visual disorders
• Psychological disorders
• Deconditioning due to debilitating medical conditions

Services Available

Physical Therapy

Rehabilitative treatment to restore mobility and independence. All treatments are geared toward helping the client attain maximum functional motor potential. The physical therapy program may consist of:

• Assessment
• Home Safety Evaluation
• Therapeutic exercises
• Transfer training
• Gait training
• Massage
• Modalities such as ultrasound
• Adaptive equipment recommendations and assistive devices for ambulation

Occupational Therapy

Rehabilitative treatment to restore maximum independence in significant roles and daily activities to include self care, work, and leisure involvement. Perceptual motor skill training and performance of activities of daily living (ADL's) are primary focus. The occupational therapy program may consist of:

• Assessment
• ADL Training (dressing, bathing, toileting, eating, meal preparation)
• Environmental adaptation and equipment recommendations
• Perceptual Motor Training
• Sensory treatment
• Muscle re-education
• Psychosocial independent living skills

Speech Pathology

Rehabilitative therapy to restore and / or relearn language and communication skills. Treatment is for those conditions that have resulted form an illness or injury which has affected speaking or hearing ability and / or language skills. The speech pathology or therapy program may consist of:

• Evaluation
• Dysphagia treatment
• Selected techniques to improve speech and language disorders
